Monticello, AR (May 12, 2025) — Mainline Health Systems, Inc. (MHSI) is proud to announce that it has been awarded the NACHC Supporting Measurement Awareness and Remote Tracking of Blood Pressure (SMART BP) grant. Valued at $40,000, this esteemed grant will enable MHSI to significantly enhance its remote patient monitoring capabilities to enhance care for patients receiving Chronic Care Management services.
The valuable grant funds will be utilized to purchase additional remote patient monitoring devices, support a portion of the Chronic Care Management (CCM) nurse salaries, and develop educational and informational videos for patients to instruct them on how to effectively use the equipment.
Out of nearly 130 applicants for the SMART BP grant, only 25 awards were granted. Each awardee scored an impressive 90/100 or higher on their proposal, making this one of the National Association of Community Health Centers’ (NACHC) most competitive grant competitions to date. MHSI is exceptionally proud of this achievement and the hard work that led to this success.
MHSI’s eligibility for the grant was largely due to the early implementation of remote patient monitoring, an initiative led by Care Management Director Heath Reep APRN, Chief Clinical Information Officer Byron Montgomery APRN, and their team. Their dedication and leadership in this field have been pivotal in enhancing patient care and ensuring MHSI’s capacity to secure this vital funding.
